This letter was read to me by a woman who sends these letters as a part of her job.
This is written so that your school does not leave out any part of your childs records,
when you request them.
You have the right to ask for and recieve your childs records,
at least once without charge. After the once your school is allowed to charge you for them.

I am the parent of {your childs name} born {birthdate},
Who is attending {school name}. I am requesting that
1 copy of my childs educational records be sent to me.
Please include all academic, health, discipline, PPT minutes,
IEP’s, confidential, and any and all evaluations performed by the school,
{Including evaluations, from any other schools my child has attended.}
or outside service providers.
Please insert a copy of this letter into my childs educational record.
